Responsibilities About the Role During visits that may last up to
one hour, our providers assess a member’s current health status,
medical history, medication adherence, social environment, and
other potential risks. This approach provides invaluable insights
into a member’s overall health that are often missed during
standard office visits. The Matrix Comprehensive Health Assessment
(CHA) enhances care quality and allows us to address multiple care
gaps in a single visit. Our providers: Conduct assessments for
adults and geriatrics, including medical history, diagnosis and
treatment, health education, referrals to physicians, case
management, follow-up, and thorough documentation in line with our
guidelines. Administer and order necessary point-of-care and
lab-based tests. Collaborate with physicians, case managers, social
workers, family members, key caregivers, and other medical
personnel as needed. Work alongside Primary Care Physicians (PCPs)
to provide patient education and follow-up as required. Deliver
services at the member’s residence, which may include their homes
or Skilled Nursing Facilities. Benefits of Working with Us

Assessments are conducted from Sunday to Saturday, 8 AM to 7 PM,
allowing you to work during the day, evening, or weekend as it
suits you. Competitive Pay – Your earnings are based on the number
of assessments completed. Increase your income by being available
for more visits and traveling to see members outside your home
area. Those averaging 20 to 100 visits monthly can earn between
$1,800 and $11,500 , depending on the per-assessment rate of $90 to
$115 based on your state. Benefits – Includes mileage
reimbursement, 401(k), employee referral bonuses, and limited
voluntary benefits. Education and Support – We invest in your
development! Along with comprehensive onboarding training, you will
be assigned a preceptor, receive personalized feedback, and have
free access to continuing education opportunities. Additional
Licensure – If business needs arise, we will assist you in
obtaining additional state licensure and credentialing in
neighboring states or others you may wish to visit while conducting
health assessments. Full-time Opportunities – Our PRN providers
have the option to transition to full-time roles supporting their
home location or a specific state or region, along with a full-time
sign-on bonus. Qualifications Skills & Experience That Stand Out A
Master’s Degree or equivalent experience with successful completion
of NP or PA licensure. Current NP or PA licensure in the state of
practice, including prescription authority or the ability to obtain
it. Board certification from NCCPA, AANP, ANCC, or AACCN in Adult,
Adult Gerontology Primary Care, Adult Gerontology Acute Care,
Family, Emergency, or Acute Care specialty. Current BLS, ACLS, or
CPR certification. Preferred one year of experience as a Nurse
Practitioner or Physician Assistant; new graduates are encouraged
to apply! A valid state driver’s license and proof of adequate
automobile insurance for your state of residence. Strong computer
skills and familiarity with health/medical record software.
