Qualifications/ Responsibilities: - Split time between OT treatment and life enrichment coordinator - Must possess an Associate's Degree in Occupational Therapy from an accredited college or university. - A current and valid license granted from the state of employment/OT/SLP and nursing assistants. - Assists in establishing an individualized treatment program with a consultation to facility staff in areas of Occupational Therapy - Aides in providing results of the evaluation and treatment objectives to caregivers, family members, and residents - Completes all documentation as required by the fiscal intermediary, within the designated time frame - Implements the treatment program based on evaluative results - Assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan Shift and Wage: - Full-Time Opportunity Available/ PRN Unavailable - Shifts Needed: Dayshift - Sign-on Bonus: $5,000 About Us: We strive to be the leader in the long-term care industry operating in 6 different states.
