Job Description Summary:

According to department policies and nursing care standards, provides professional nursing care services to patients in the home. In doing so, demonstrates the knowledge and skills necessary to provide patient care. Responsible for assessing, planning, implementing and evaluating nursing care provided to assigned patients. The following are general characteristics of this job, although duties may vary.

Minimum Qualifications:

Associate's Degree (Required)DL - Driver's License - Department of Motor Vehicles, RN - Registered Nurse - Ohio Board of NursingDemonstrates knowledge and skills related to the assessment and care of the Home Health patient. Valid State Driver's License - and maintains automobile liability insurance. Minimum one-year hospital nursing Experience . Preferred Previous Experience in home health. 3 years of hospital/home health Experience .
